All Students will need to get a yearly physical exam turned in to your Athletic Trainer of Middle School campus coordinator.

*All students will be required to complete all online paperwork.

Athletic/Fine Arts Participation Packet

• All students must have an Athletic Physical, medical history, and all electronic forms completed prior to participating in an athletic class, practice, performance or event.

• You will need your student’s ID number prior to starting completion of the forms. Names must be entered EXACTLY as they are in Skyward including dual last names or hyphenation etc.

1. Select Download and Print to retrieve the UIL physical and medical history form. This will be completed and turned into your Athletic Trainer or Campus Coordinator at the Middle School.

2. Select Electronic Participation forms. Complete each form online. Each document must be signed. This can be done by clicking in the box and holding your mouse down. If you make a mistake, select refresh below the signature box to reset the signature box.

3. After you complete all forms your will receive a confirmation email that they have been submitted.

IF THE SCHOOL IS INCORRECT IN THE DROP DOWN BOX IT WILL SELF CORRECT DURING THE SUMMER. IT IS OK TO COMPLETE THE FORMS
